Exciting opportunity in Overlake's brand new Off-Campus Emergency Department.
Overlake is preparing to open our first off-campus ED (OCED) facility in Lynnwood this fall. This new OCED will provide 24/7 ED-level care in a highly collaborative, fast-paced setting. It will be based on the model MultiCare has used to open highly successful OCED locations across Washington State.
The Social Worker works in partnership with the treatment team to assure continuity and coordination of care for patients, families, or legal representatives.  This position is responsible for risk screening, providing services and consultation to patients/families regarding social, environmental, psychologic, financial and cultural factors related to illness and hospitalization, supporting continuity of care, and providing discharge/transition management.  The Social Worker also assesses for mental health and substance use crisis, provides grief and loss support, coordinates with community resources, and in collaboration with the Care Manager manages appropriate utilization of resources and length of stay.

0.5 FTE / 20 hours per week
Shift Lengths: 6 & 7 hours
Qualifications:
 
  • Master’s degree in social work, mental health counseling or related field required.
  • Candidate must possess one of the following licenses from the state of Washington:  Licensed Independent Clinical Social Worker (LW), Licensed Advanced Social Worker (LW), Licensed Mental Health Counselor (LH), or Licensed Marriage and Family Therapist (LF).
  • Minimum of two years’ experience is required.
  • Requires assessment and care planning skills, broad knowledge of the community resources, medical terminology, hospital systems, insurance guidelines, financial issues, and risk management issues.  Effective verbal and written communication skills and computer skills. An understanding of the issues that are involved in the documentation and continual quality improvement process of patient care in the hospital setting preferred.
